From the practitioner: About My Practice
Please prepare a timeline of your health by year and event. List significant symptoms you are struggling with and provide a summary of who you saw for your care, what treatments were prescribed and whether or not you feel it helped you. Please make a list of your prescription medications and any over the counter products that you consume. Please bring the original containers to your appointment (both prescription and over the counter). Please make a list by year for you employment history as well as any hobbies. Why is this important? It assists in creating a picture of your life and possible environmental exposures that may provide clues to the symptoms you are experiencing. Your environment and your genetics interact.
From the practitioner: What Patients Can Expect
Assistance with navigating the complex web and connections to explain where you are today with your health and how we can move forward to improve your health. As a physician, I first ensure that all necessary standard laboratory and radiological investigations have been done (and if not, I coordinate them), and then add integrative tests as required. Functional medicine combines all modalities in the best interest of the patient. This integrative model has been beneficial for patients.
