From the practitioner: About My Practice
Chosen Wellness Group is a multi-disciplinary medical group offering patients functional medicine with conventional primary care and health coaching. Our team works together to diagnose and treat the underlying root cause of your problem. Patients commonly come to us with digestive problems, fatigue, stress, headaches, hormonal imbalance, and autoimmune and chronic diseases. They are tired with the traditional "pill for ill model"and want to resolve the cause of their health problems – that is our specialty.

Every patient is offered an initial telemedicine consultation. Next, an extensive medical history/physical exam gathers the information needed to help customize a treatment plan. Understanding the root cause of your health problems and putting you first is our priority.
